What different types of implants are there?

Round implants

Round implants stand for a rather full, “loud” décolleté and are characterized by a central bulge that makes the décolleté appear fuller. The breast is given maximum fullness, profile and volume. Due to the “unnatural” shape, the wheels or contour can be clearly visible with large implants. The implants can be placed above or below the pectoral muscle.

The teardrop shape is similar to the natural breast shape. The upper part of the breast, i.e. above the nipple, is flatter here. The center of gravity of the implant forms the lower area. This guarantees a harmonious transition from the ribcage to the breast. These implants result in a natural appearance and are typically placed under the pectoral muscle.

Ergonomic implants

Ergonomic implants are a special type of implant. Due to their special shape, they appear round when lying down and anatomical when standing up. They are usually placed over the pectoral muscle. They are significantly softer than conventional implants.

Who is breast augmentation with silicone suitable for?

In principle, this operation is suitable for any woman of full age who has completed breast growth. The desire for a full décolleté with a harmonious breast shape is clearly in the foreground here. Women who have suffered from flat, only moderately pronounced breasts since puberty are particularly keen to have this type of operation. However, pronounced asymmetries or “empty breasts” after weight loss or childbirth are also common reasons for choosing this procedure.

When is the best time?

Breast augmentation is an aesthetic operation. In general, there is no time pressure. Every woman will therefore choose the time for breast augmentation for her own reasons. The desire is often very strong after reaching the age of majority or after completion of breast growth and after pregnancy or weight loss.

Who is it not suitable for?

The insertion of silicone implants is a quick and efficient solution to generate more volume and fullness. However, there are women for whom silicone is “never” an option for moral reasons. If breast augmentation is nevertheless desired, there is the option of breast augmentation with autologous fat. Disease-related changes due to pronounced abnormalities or tumors are also unsuitable starting situations.

How does the operation work technically?

Breast augmentation with silicone consists of a total of two stages, all of which take place during a surgical procedure and last around one hour. These include the creation of an implant cavity and the insertion of the appropriate implant.

At the beginning of the operation, a 4 cm incision is made under general anesthesia. In most cases, this is located in the underbust crease. This incision is used to expose a space that is customized for the selected implant. Depending on your anatomy, this is located above or below the pectoral muscle. In the second step, the implant is carefully inserted and the wound is closed. In rare cases, drains are inserted, which are removed on the first day. The skin is closed with a thin suture material. No visible suture remains.

Surgical and conservative alternatives

There are various conservative and surgical alternatives for many operations. It is therefore necessary to consider these carefully before any surgical intervention. If the use of a scalpel can be dispensed with due to conservative alternatives, these are of course preferable.

In the case of breast augmentation, however, the options with non-surgical alternatives are extremely limited. Although special vacuum treatments have shown a small increase in volume in studies, this required wearing the corresponding vacuum pumps for three months and 20 hours. This treatment method was rarely chosen and is therefore not relevant. Another surgical alternative is breast augmentation with autologous fat.

What type of anesthesia is necessary?

You will be under general anesthesia during the operation. You will not feel any pain or be aware of the procedure. Sedation, as with minor operations, is not provided here. The biggest difference between anesthesia and sedation is the use of a ventilator.

Can the procedure be combined with other procedures?

Breast augmentation can be combined with many other procedures. Patients with sagging skin often opt for an additional breast lift. This is particularly useful for loose connective tissue and excess skin. Small autologous fat transplants to fill volume defects on the face or lips are also very popular.

Your breasts will take about this time to reach their final position. Initially, you will notice that the scars in the underbust fold are still slightly red. This redness will fade over time. Depending on the patient’s wishes and the implant chosen, you can expect a more natural or unnaturally large result.

Is this operation performed on an outpatient or inpatient basis?

We perform breast augmentation in a private hospital. It is usually an outpatient procedure. A total of 3-4 hours should be planned. Even if the actual operation time is 1.5-2 hours, the time for preparation and follow-up must be taken into account. For example, sleeping pills must be taken. You are welcome to stay in hospital until you feel fit again. Immediately after waking up, you may experience slight circulatory weakness. Active participation in road traffic should be avoided at all costs for the first 12 hours. An inpatient stay can of course be planned on request.

Risks and complications

During surgical procedures, skin and tissue as well as underlying structures are treated with sharp instruments. Despite all caution, complications of varying severity can occur. It is important that you are adequately informed about all possible risks and complications before a planned operation. During the initial consultation, we will go through a corresponding information sheet with you.

Minor complications

Breast augmentation is a procedure with relatively few complications. If performed carefully, the risk of permanent consequences is very low. As with all surgical procedures, there may be minimal pain, comparable to moderate muscle soreness, as well as swelling and hematomas. The placement of the implants determines the severity of the pain. Cutting the muscle fibers, e.g. with anatomical implants, leads to slightly longer pain. However, this should decrease significantly after approx. 2-3 weeks.

Rare major complications

There is an increased risk of secondary bleeding within the first 12-24 hours after the operation. The inserted drains serve as a control system with the tight bandage. In addition, complications can arise in the early phase after the operation due to changes in position caused by pressure or incorrect loading. The special bra will provide you with support here. Other rare complications, such as the development of capsular fibrosis, ruptures or BIA ALCL (breast implant-associated anaplastic large cell lymphoma, a rare subtype of non-Hodgkin’s lymphoma), will be discussed with you during a detailed consultation.

What to do in the event of complications?

If, contrary to expectations, complications occur, we will not leave you to deal with them alone. After the operation, you will be given an emergency number where you can reach us 24 hours a day. During the routine check-ups following the operation, we will discuss the progress and further procedure with you in detail.

Healing and progression

As soon as patients have decided to have an operation, there is great anticipation of the result. However, depending on the procedure, there is a long way to go. You will need to be patient until the final result is achieved. If you know what to expect before your planned operation, this path will be easier.

How long does the healing process take?

The implants require approx. 4-6 weeks to heal completely. Any form of sport or pressure/shock should be avoided, as strong movements can lead to loosening and possibly rotation of the implants.

Swelling, hematomas and slight discomfort may occur in the first 24-48 hours. It is normal to take mild painkillers.

Most patients rest on the first day after the operation and take short walks. After 48 hours, showering with “shower plasters” is possible.
If you do not have to do any heavy physical work, you will probably be able to return to work from the 7th-10th day after the operation.

After approx. one month, the majority of patients feel as they did before. Slight bruising and any swelling will have largely disappeared. Over the next few months, your breasts will assume their “final” position and sag by around 0.5-1 cm.

After 6-8 weeks, all sporting activities are possible again. Exceptions to this are competitive athletes who put a lot of strain on their chest muscles.

Of course, these time recommendations are guidelines based on our experience, which sometimes require individual adjustment.

Is the treatment painful

Breast augmentation with silicone implants is not very painful. You will not feel any pain during the operation. Immediately after the procedure, the painkillers will continue to have an effect, so that you will probably only feel moderate pain in the breast area in the evening of the day of the operation. Immediately after the operation, we will put on your compression bra. This prevents the implant from moving under the breast and thus ensures optimal healing.

What should patients plan?

A surgical procedure requires a certain amount of planning and organization not only for the surgical team, but also for the patient, both before and after the operation. It is important that you are well prepared and know what to expect. This allows you to concentrate fully on enjoying the results.

Before any breast surgery, we recommend a gynecological check-up to ensure that there is no underlying breast disease. As soon as you have decided to have the operation after a detailed consultation and anamnesis, you should plan the time after the operation with your work or family accordingly. Although a longer period of absence is not to be expected, it is often the case that you will be slightly restricted in your movements.

Plan the time after your operation

Although you will not have to expect long periods of absence after breast augmentation, you should still plan your time afterwards. It may be temporarily more difficult to pursue social or private commitments. You should also plan your “time off” with your employer and family in the run-up to surgery. It is not uncommon for patients to experience an emotional change after an operation due to the physical change. If you are expecting a stressful period at work, we recommend that you postpone the operation if possible.
